Telma R Tablet 5mg is an anti-hypertensive medicine which helps to reduce blood pressure by widening the blood vessels and used in the treatment of diabetic nephropathy – in hypertensive patients with type-2 diabetes mellitus. The tablet is also used in patients with congestive heart failure and helps in survival after a heart attack. However, the uses mentioned here are not exhaustive. There may be other conditions for which this medication may be used upon doctor’s discretion.

The active ingredients of Telma R Tablet 5mg are Telmisartan 40mg and Ramipril 5mg.

Telmisartan acts by selectively antagonizing Angiotensin-2 AT 1 receptor. With this, it reduces vasoconstriction and blood pressure.
Ramipril acts by interfering with the conversion of angiotensin-1 to angiotensin-2 by this, a rise in blood pressure slowly comes down.

Patients with the following conditions are suggested to seek advice from their health care professionals before taking the Telma R Tablet 5mg:

  • Hypersensitivity to any of the components used in the tablet such as Telmisartan and Ramipril
  • AEC inhibitor angioedema history
  • Angioedema, hereditary

Side and Adverse Effects

The following side effects may or may not occur during the usage of this Telma R tablet 5mg. It is generally well tolerated when taken in the prescribed dosage guidelines. More common ones are generally mild and may include:

  • Back pain

How to Use

How to Use

Do not exceed the dose prescribed by your doctor. Swallow the tablet whole, immediately after opening the tablet from the strip. Do not break or chew the Telma R Tablet 5mg.
Avoid missing or forgetting a dose. If you do, take it as soon as you remember; but if it is time for your next dose, skip it and take it according to your normal schedule. Don’t swallow two doses at the same time which is dangerous to your health.
This medicine may be taken with or without food.
Take the dosage as directed by your doctor or follow the directions.

Overdosage

Avoid taking more than the prescribed dosage. In case of an accidental overdose, promptly seek medical advice. Depending on the dosage, symptoms may vary but you may experience one or more of the following signs or symptoms:

  • Hypotension
  • Bradycardia
  • Tachycardia
  • Dizziness
  • Acute renal failure and serum creatinine
  • Electrolyte disturbance

Warnings and Precautions

Take the Telma R Tablet 5mg only on the advice of a medical practitioner. Dosage adjustment or complete avoidance of the medication may be required in the following conditions:

Kidney:

Caution advised among people with renal problems. If you have any, it is highly recommended that you consult your doctor before the consumption of Telma R Tablet 5mg.

Liver:

Caution advised among people with hepatic-related problems and dosage adjustment may be needed. If liver impairment is severe this medicine is contraindicated in such patients so it is highly recommended that you consult your doctor before the consumption of this Telma R Tablet 5mg.

Pregnancy:

This medicine is not recommended if you are pregnant or planning to get pregnant because there is positive evidence of human fetal risk.

Lactation:

Do not take this medicine if you are a nursing mother because it is not known whether this drug could harm a breastfed infant or not.

Alcohol:

Do not drink alcohol if you are taking this medicine because alcohol may decrease your blood pressure and increases the side effects of Telmisartan.

Drug Interactions:

Caution always advised when used with other drugs such as Digoxin, Lithium, Diuretic or water pill, Ramipril or other BP medicines and NSAID’s, etc. However, inform your doctor about your previous and current history of medicines which you have used.

Precautions
  • Keep the medicine out of sight and reach of children.
  • Keep the medicine in room temperature away from heat and moisture.
  • Do not use the medicine beyond its expiry.
  • Do not use Telma R Tablet 5mg if the tablet strip is torn or the tablet is broken.
  • Avoid sudden or too quick sitting or lying postures to prevent a fall because the tablet some times lead to postural hypotension.

What is Ramipril for:

This medication is an angiotensin-converting enzyme (ACE) inhibitor, used alone or in combination with other medications to treat high blood pressure. It is also used to reduce the risk of heart attack and stroke in patients at risk for these problems and to improve survival in patients with heart failure after a heart attack.

How does Ramipril work:

Ramipril lowers blood pressure by lowering a strong chemical in the body. It helps the heart work better. It helps blood flow.

How should Ramipril be used:

Take as you have been told, even if you are feeling better. Take with or without food. Take with food if it causes an upset stomach. You may sprinkle contents of capsule on applesauce or in apple juice. Do not chew. Follow the diet and workout plan that your doctor told you about.

Common side effects of Ramipril :

Feeling dizzy. Rise slowly over a few minutes when sitting or lying down. Be careful climbing. Headache. Bad taste in your mouth. This most often goes back to normal. Cough.

What do I do if I miss a dose

Take a missed dose as soon as you think about it. If it is close to the time for your next dose, skip the missed dose and go back to your normal time. Do not take more often than every 12 hours unless told to do so by your doctor. Do not take 2 doses at the same time or extra doses. Do not change the dose or stop this drug. Talk with the doctor.

What precautions should I take when taking Ramipril :

If you have an allergy to ramipril or any other part of this drug. Tell your doctor if you are allergic to any drugs. Make sure to tell about the allergy and what signs you had. This includes telling about rash; hives; itching; shortness of breath; wheezing; cough; swelling of face, lips, tongue, or throat; or any other signs. If you have high blood sugar (diabetes) and are taking aliskiren. If you are pregnant or may be pregnant.

When do I need to seek medical help

If you think there was an overdose, call your local poison control center or ER right away. Signs of a very bad reaction to the drug. These include wheezing; chest tightness; fever; itching; bad cough; blue or gray skin color; seizures; or swelling of face, lips, tongue, or throat. Signs of infection. These include a fever of 100.5°F (38°C) or higher, chills, very bad sore throat, ear or sinus pain, cough, more sputum or change in color of sputum, pain with passing urine, mouth sores, wound that will not heal, or anal itching or pain. Very bad dizziness or passing out. Trouble breathing. Too much sweat, fluid loss, throwing up, or loose stools. May lead to low blood pressure. A big weight gain. Swelling in your legs or belly. Cough that does not go away. Dark urine or yellow skin or eyes. Any rash. Side effect or health problem is not better or you are feeling worse.

Can I take Ramipril with other medicines:

Inform your doctor if you are taking any of these medicines: - a mood medicine called lithium. - aspirin. - other medicines for blood pressure or heart conditions. - a type of painkiller known as NSAIDs, for example, diclofenac, mefenamic acid, ibuprofen. - diabetes medicine. - potassium supplements (either as a medicine or as a salt substitute). - Always inform your doctor and pharmacist if you are taking any other medicines, including herbal tonics, supplements and medicines that you buy without a prescription.

Are there any food restrictions

Cut down on your salt intake -- this may help lower your blood pressure and improve your overall health. Speak to your doctor or dietitian about ways to cut down on your salt intake. - Avoid drinking too much alcohol. Speak to your doctor if you have a drinking problem.

How do I store Ramipril :

Store in a cool, dry place away from the reach of children. - Medicines must not be used past the expiry date.
